“An approach that supports individuals to take greater control of their own health & wellbeing. It does this by connecting people to activities, groups, & services in their community. The goal is to meet their practical, social, and emotional needs based on what is most important to them.”
Example can include:
•Supporting someone to improve their wellbeing by getting involved in activity that is meaningful & helps to build confidence.
•Helping someone who feels lonely & isolated to join a group, class, community project or sports activity based on what works for them.
•Connecting someone struggling with financial stress to a service that helps with managing debt or claiming benefits.
•Supporting someone with dementia to join a dementia choir, enabling them to maintain a sense of social connection.
•Working with someone with physical health issues who wants to increase their activity levels.
•Being a listening ear to someone and supporting them to explore what matters to them and changes that they would like to make to improve their overall wellbeing.

We are a group of partners working in St Helens who are developing a social prescribing network where people whose job involves the task of social prescribing can join and share resources, knowledge, gain peer support and training/development opportunities through online and face to face sessions.
